Cityblock co-founder and CEO Dr. Toyin Ajayi saw a healthcare system that was failing her patients. So she set out to change it. That’s how Cityblock was born–to radically change how healthcare works for the people who need it most.

When Cityblock members choose to work with us, they keep the providers and network they have through their health plan. But they also get us as a team by their side. Here’s one example of what that team can look like.
Community Health Partner (CHP)
“I’m your first stop when you need something or have questions about your health. I can connect you to social services for childcare support, housing, fresh food—you name it.
Medical Provider
“I diagnose and treat injuries or illnesses. I can provide ongoing care, especially for complex or chronic conditions.”
Therapist & Mental Health Provider
“I support you when you’re stressed out or in crisis. I help provide treatment for substance use disorders, addiction issues, and other conditions.”
Clinical Pharmacist
“I’m licensed to prescribe medications and evaluate how well they’re working. I can consult with you on what medicine might help you.”
